We, RARA AMORES, of legal age, Filipino, widow and RARO RENDON, of legal age, Filipino, married and both residents of Earth Village, Metro Manila, Philippines, after having been sworn to in accordance with law hereby depose and say:

1. That we personally know the person of EARL NABOA AYUSTE JR. who was born on April 30, 1964 in Nabunturan, Davao, he being the legitimate son of our close friend and neighbor, the Spouses Rafael Ayuste Sr. and Cher Naboa (both deceased) ;

2. That in the Certificate of Live Birth of EARL NABOA AYUSTE JR. filed in the National Statistic Office his surname and the surname of his father were erroneously entered and spelled as “ AYOSTE ” while his middle name and of his mother were also erroneously entered as “NABUA” ;

3. That the true and correct spelling of the middle name of EARL N. AYUSTE is “ NABOA” and his surname true and correct spelling is “AYUSTE” ;

4. That the name “EARL NABOA AYUSTE JR.” and “EARL NABUA

AYOSTE JR. “ refers to one and the same person;

5. That we are executing this affidavit to attest to the truth of the foregoing facts.
