         This is an example of an affidavit of support executed by the father.  This sample affidavit of support includes recognition by the father that the child being carried by a woman is his.

AFFIDAVIT OF SUPPORT

            I, (insert name of father of the child)  , Filipino, of legal age, married   and a resident of  (insert address) , after having  duly sworn to in accordance with law, depose and say that:

1.      I  know the person of  (insert name of mother of the child) and that I acknowledge the baby she is presently carrying as mine;

2.      Because of this , I pledge to provide financial support to (name of mother of child) during her entire pregnancy  in the    amount of (insert amount in words and in numbers) monthly and until she gives birth;

3.      I also vow to support her financially during her delivery;

4.      Should the needs of the baby increase and should my means improve, I promise to increase the amount of financial support subject to another agreement which I and (name of mother of child) shall enter into in the future;

5.      I am executing this affidavit to attest to the foregoing facts and for  whatever legal intent and purpose this may serve.
